Pre-Registration
Pre-registration for the Conway Regional Medical Center is currently available in-person or online. For OB patients, you can save time and get your information into the system before you come to the hospital to have your baby by pre-registering at 30 weeks before your expected due date for delivery. OB patients can pre-register in-person, online, or by mail.
You may pre-register at the 1st floor registration desk in the east lobby between 8:00 am - 5:00 pm Monday through Friday.
OB patients may pre-register at the registration desk on the 1st floor of the women's center 8:00 am - 5:00 pm Monday through Friday.
A photo I.D. and insurance card will be required.
To save you time, we offer the ability to submit your paperwork prior to your scheduled appointment.
OB patients may pre-register by completing the OB-PreAdmitForm2020 and mailing it to the following address with a copy of your photo I.D. and insurance card.
